A lot of men come in feeling good about their chest overall but bothered by one small thing they can’t unsee. Their nipples stick out more than they should. They look thicker, longer, or more noticeable than the rest of the chest, especially in T-shirts, tank tops, or swimwear. Most of the time the pecs themselves look fine. Flat. Solid. Muscular. The nipples just don’t match.
Male nipple reduction is for men who want to fix that specific issue without changing anything else. It’s not about making the chest look different. It’s about getting the nipples back into proportion so they stop drawing attention.
Dr. Douglas Steinbrech is board-certified and works only with men. Because his practice focuses exclusively on male patients, he sees this concern regularly and understands the small details that matter on a male chest.
Understanding Enlarged Nipples in Men
One thing that comes up a lot is confusion about what’s actually causing the problem. Men will often say they have “puffy nipples,” but that can mean two different things.
Sometimes it really is the nipple. The center part sticks out too far or looks wider than normal. The darker skin around it stays pretty flat. It usually looks worse when you’re cold or after stimulation. When that’s the case, nipple reduction is the correct procedure.

Other times the entire darker area pushes outward. The nipple itself doesn’t project much, but the whole areola looks rounded or raised. That usually comes from excess breast tissue underneath. When that’s the issue, nipple reduction won’t fix it. That’s treated with male breast reduction for gynecomastia.
Getting that distinction right is important. Treating the wrong thing doesn’t help and just creates frustration.
How Does Nipple Reduction for Men Work?
When nipple reduction is the right option, the procedure itself is straightforward. It’s done in the office under local anesthesia. You don’t have to go fully under. Once the area is numb, excess tissue is removed carefully so the nipple ends up smaller and flatter but still looks natural. Nothing extreme. Nothing that looks overdone.
Most procedures take around half an hour, sometimes a little longer. Stitches dissolve on their own. Ointment and small bandages go on, and you head home the same day.

Because the work is limited to the nipple, most men say recovery is easier than they expected. There’s some soreness and swelling at first, but it’s usually mild. Tylenol is typically enough. Heavy lifting and hard workouts should wait about a week. Chest workouts usually wait closer to two weeks. Showering is fine after the first day, just gently. The stitches usually disappear within ten to fourteen days.
Scarring is small and tends to fade quickly. Nipple skin heals well on its own.
Are You a Candidate for Nipple Reduction?
Before surgery, patients are asked to stop aspirin, ibuprofen, and similar medications for a couple of weeks to reduce bruising. Smoking or vaping should also stop before and after surgery since nicotine slows healing. If someone gets sick close to the procedure date, it can be moved. That’s not a problem.
This procedure works best for healthy men whose concern is really about nipple size or projection, not fullness of the entire chest. Men with nipple piercings usually need to have them redone later. During consultation, Dr. Steinbrech evaluates everything in person to make sure nipple reduction alone makes sense.

Most men naturally have nipples that sit flat and don’t stand out. When they’re oversized or raised, even a lean chest can look softer than it should. Reducing them back to a normal size brings everything into balance. Shirts fit better. You stop thinking about it. That’s usually the biggest change.
Schedule a Nipple Reduction Consultation
Even though this is a small procedure, precision matters. Small adjustments can make a big visual difference. Working only with male patients has given Dr. Steinbrech a clear sense of what looks right on a male frame and what doesn’t.
If this is something you’ve been thinking about for a while, a consultation is the next step. It’s the easiest way to figure out whether nipple reduction or gynecomastia surgery is actually the right solution for you.
